Cutaneous draining sinus tracts of dental origin often are a diagnostic challenge. Early correct diagnosis and treatment of these lesions can help prevent unnecessary and ineffective antibiotics therapy or surgical endodontic treatment. Single sitting root canal treatment can be an answer in many endodontic cases provided the cases are properly selected and the technique is carried out with clear understanding of the basic concept of different phases of endodontic treatment.

In this case we performed single sitting root canal treatment in a tooth with draining sinus and well defined periapical pathology. A 3-month follow up showed successful healing.
